Hazel is an AHPRA registered Occupational Therapist with 30 years of experience in hospital and community settings. In 2019 she set up Equipped to Function to provide quality-driven, compassionate, personalised Occupational Therapy services across the community aged care, rehabilitation and disability sectors.

Jonathan Lee - Occupational Therapist
Jonathan is a dedicated Occupational Therapist with extensive experience across a diverse array of settings, including aged care, community-based rehabilitation, public outpatient services, and inpatient neurological rehabilitation.
Passionate about delivering high-quality Occupational Therapy services, Jonathan is committed to enhancing the quality of life for his clients by helping them achieve positive outcomes.
Jonathan possesses expertise in assistive technology, equipment prescription, and home modifications. His special interest lies in working with individuals diagnosed with neurological conditions, ranging from post-stroke rehabilitation to Parkinson's disease.
Jonathan is registered with the Australian Health Practitioner Regulation Agency (AHPRA) and holds certifications in the Montreal Cognitive Assessment (MoCA) and LSVT BIG, a specialised program for Parkinson's disease management.
Anita Green - Occupational Therapist
Anita brings over 17 years of extensive experience as an Occupational Therapist across diverse settings and roles, ranging from NDIS, Occupational Rehabilitation to Acute Care, and Multi-Trauma Rehabilitation both in Australia and the UK.
She maintains registration with AHPRA and membership with OT Australia. Additionally, Anita is FIM accredited and holds certification in MoCA, underscoring her commitment to professional excellence.
With a refined focus on Equipment Prescription and Home Modifications, Anita has developed an affinity for Aged Care. She adeptly combines clinical expertise with compassion, tailoring interventions to meet individual needs and enhance independence and quality of life.
Anita also enjoys sports, humour, and loves to see others achieve their goals and independence, fostering a holistic approach to her practice.
