Hazel is an AHPRA registered Occupational Therapist with 30 years of experience in hospital and community settings. In 2019 she set up Equipped to Function to provide quality-driven, compassionate, personalised Occupational Therapy services across the community aged care, rehabilitation and disability sectors.

Ina Davies - Occupational Therapist
Graduating with a Master of Occupational Therapy from the University of Canberra, Ina has dedicated her career to compassionately serving communities across metropolitan and regional areas, both at home and abroad.
Focusing primarily on assisting the aging population, Ina specialises in providing essential services such as assistive technology, home modifications, and equipment prescriptions. Her passion lies in enhancing the wellbeing and safety of adults as they strive to maintain independence within their own homes.
Her particular interests encompass seating and pressure care, and a commitment to ensuring her clients receive the utmost comfort and support.
Ina is registered with APHRA.
Anita Green - Occupational Therapist
Anita brings over 17 years of extensive experience as an Occupational Therapist across diverse settings and roles, ranging from NDIS, Occupational Rehabilitation to Acute Care, and Multi-Trauma Rehabilitation both in Australia and the UK.
She maintains registration with AHPRA and membership with OT Australia. Additionally, Anita is FIM accredited and holds certification in MoCA, underscoring her commitment to professional excellence.
With a refined focus on Equipment Prescription and Home Modifications, Anita has developed an affinity for Aged Care. She adeptly combines clinical expertise with compassion, tailoring interventions to meet individual needs and enhance independence and quality of life.
Anita also enjoys sports, humour, and loves to see others achieve their goals and independence, fostering a holistic approach to her practice.
Rachael Vallentine - Occupational Therapist
Rachael is a dedicated and adaptable Occupational Therapist with over 25 years of experience delivering high-quality occupational therapy services to adults across various settings including community, day program, and inpatient care. Rachael possesses a breadth of expertise in the aged care, rehabilitation, and disability sectors.
Rachael has developed skills in the assessment of home modifications and equipment prescription, functional task analysis, client-centred goal setting, and comprehensive treatment planning and delivery.
Her professional interests are centred around seating and pressure care management, cognitive impairment interventions, and precise equipment prescription. With a commitment to professional excellence, Rachael remains dedicated to enhancing the quality of life and promoting independence for individuals under her care.
Rachael holds APHRA registration.
